We ask you to send information with your request for remission. It depends on your situation whether you need to complete the regular form, or an abridged version.
You will be given the regular form if:
- More than 2 people aged 27 or over live at your address;
- You are self-employed and/or a student.
Do neither of the two situations above apply to you? Then you will receive an abridged form.
On the abridged form, there are 3 questions where we may sometimes ask for additional information
We will ask about your situation to see if we need additional information from you. We ask you whether:
- You receive income from room rentals. If so, we will ask you for a copy of the rental agreement.
- You pay or receive alimony. If so, we will ask you to send a bank statement from the previous month showing the amount of alimony and your income.
- You own your own home. If so, we will ask for 2 documents;
- A copy of the most recent annual mortgage statement.
- A copy of the bank statements showing the debits of the monthly mortgage payments. Do you have a insurance policy linked (pledged) to the mortgage? Please include a financial statement of this policy.
On the regular form, we ask for additional information on 4 issues
3 of the questions on the regular form are the same as the abbreviated form. Above, you can see in which situations we ask for extra information from you. On the regular form, we will also always ask for:
- Copies of statements of your bank accounts for the past two months. These are statements of all your bank accounts, savings accounts, savings policies and accounts abroad. The statements must clearly show the name, date, credits and debits and current balance.
- A copy of your health insurance policy document showing the amount of premium you pay.
- A copy of the decision of the Dutch Tax Office for prepayment of child budget, childcare allowance, rent supplement and/or healthcare allowance.
- A copy of the provisional tax restitution document, if you receive this.

Use your Dutch Digital Identification (DigiD) to log in to 'Mijn Loket'. Please enclose all requested information straight away. If you do not provide all the information, we cannot process your request. In that event, your request will have to be rejected and the tax assessment must be payed. You will always receive a letter about our decision on your request for remission.
